Ladder characters simply have a little box checked that says 'ladder character'. Characters with this option are tracked by b.net and rated against every other character. The elite are displayed on b.net for all to see. There are also ladder only items that can only be found playing on ladder.

Ladder characters and non-ladder characters can't play together.

The ladder also has seasons. The seasons are about 6 or 8 months (can't remember) and then the ladder resets and a new season starts. When it resets, all Ladder characters are turned into Non-Ladder characters (leaving their items intact, which can lead to a very small amount of Ladder only items on Non-Ladder), and only new characters can be made on the Ladder. So everytime the Ladder resets, it's like a whole new battle.net, everybody is starting at lvl1 and nobody has great items. There's no rushing cause everybody is at about the same lvl. There's also generally no dupes on the new Ladder for a while, cause no one has anything to dupe. Great fun.
